Wide complex tachycardias (WCT) includes a broad spectrum of supraventricular and ventricular tachycardias with QRS complex duration of 120 ms or more. The differential diagnosis is essential for the correct treatment and prognosis of patients. Several algorithms are used for their analysis, which makes it possible to diagnose them in most of the cases. We present a clinical case of a patient with a tachycardia with QRS complex duration between 120 and 140 ms, with right bundle branch block morphology. Based upon the classic criteria and algorithms and the imaging examinations used we accepted the diagnosis of idiopathic left ventricular tachycardia. It was confirmed with an electrophysiology study and a successful catheter ablation was performed.
