Persistent left superior vena cava (PLSVC), even if it`s rare, it imposes a difficulty for the implantation of cardiac rhythm devices. Standard implantation techniques are hardly applicable and often should be modified in a way that they become potentially dangerous. We report case of a patient with PLSVC in whom we successfully performed a left sided implantation of dual chamber pacemaker using the “loop and drop- down” technique for implantation of the right ventricle lead and successful implantation of the right atrial lead in the right atrial appendage. Adherence to the standard implantation techniques in patients with PLSVC is hard, but possible. The techniques minimize the complication rate and chances of misplacement of the leads.
